I have agreed to serve as a mentor for the following student during the activity phase of the student’s project. I understand that I am responsible for monitoring the student’s progress, meeting with him or her to discuss problems and necessary adjustments, and certifying the hours spent on the activity. I also agree to write a letter for the student’s portfolio explaining what the student has done under my direction.
