These immunizations are required by the state of Montana.

All new enrolled students grades K-12 must meet the age appropriate immunization requirements before attending school unless an exemption or conditional is in place.

A claim of religious exemption for immunizations must be notarized annually and maintained on the Religious Exemption form. The form must be provided to the school prior to each school year by the student’s parent/legal guardian. If a student has contracted a disease or is exposed to a disease, the student may be excluded from school and any school related activities by the local health officer or the state department until the excluding authority feels that there is no longer a risk of contracting or transmitting that disease.

1. DPT/DTaP (Diphtheria/Pertussis/Tetanus):

Student needs a minimum of 4 doses before entering kindergarten. One of these doses needs to be given after the child’s 4th birthday.

2. IPV/OPV (Polio):

Student needs a minimum of 3 doses before entering kindergarten. One of these doses needs to be given after the child’s 4th birthday.

3. MMR (Measles, Mumps, Rubella):

Student needs a minimum of 2 doses before entering kindergarten. First dose has to be given after the 1st birthday.

4. Td booster (tetanus/diphtheria):

Tdap is recommended as it contains pertussis booster also. One dose is required before entering 7th grade.

5. Tdap booster shot(tetanus/diphtheria/pertussis):

One dose is required prior to entering the 7th grade.

6. Varicella (Chicken pox):

2 doses required.  The first vaccination must be given on or after the 1st birthday.
